I Need Help with an Image Uploder Script

Can anyone help me all i need is a n image uploader script like this is what im looking for.

Once they upload the image it show them the image they have uploaded and the link to the image thats all im looking for and i cant event find that lmao.

I can style it up i just need the script can anyone help me out?
