    No, but if you sell above a certain amount, you have to be VAT registered.

    If you're wanting to become an Ltd. you need a memorandum of association and articles of association. Then two forms known as Form 10 and Form 12.
    The forms to do this are available from Companies House (Saurav has provided link).

    Yes only if you want to be a limited company which then protects you against personal debt. You can be a sole trader without going through all that.
