My toenails come off many times. It kills doesn't it. No need really to go to the doctor. It could get infected - mine didn't. It just hurt wearing socks and shoes, just refrain from touching it. Put a plaster over it to fight infection. And just don't panic!

ergh no, don't post pics. As others have suggested, put a plaster on it and keep the area clean.
