Nope not a glitch, I've been in a case where 8 guardians are deciding it before. I'm not sure how it happens, but it's not a glitch I don't think

I think it allows more if a few people pick it up at the same time and if there is no majority in a handful of cases, otherwise it goes to guardians. Originally Posted by Lewis

Yes if it's undecided like two of "OK", two of "Bad" and one of "Serious" then it's opened to a further 5 guardians.
