I always thought it meant just taking a break from the relationship for whatever reason they've decided, usually because something isn't right and they need time apart to figure things out. Maybe others have different opinions though.

To me a break usually means that you're giving each other space so you can think about what to do next, at least that's what people have explained to me about the whole thing. I personally don't like the term "break" because you're either together or not, there's no inbetween at all.
